All members of the Arizona Veterans Hall of Fame Society are invited and encouraged to complete the AVHOFS Member Information Form below.  The information collected will be utilized to better communicate and serve our members, while also giving you an opportunity to provide information to the Society regarding your time and talents, which committees you’re interested in, or the programs/projects you’d like to participate in or volunteer for.

COMMITTEES:  The Society has both Standing and Special Committees per our Bylaws. Standing Committees include: The Hall of Fame Event Planning Committee, the Patriotic Awards Luncheon Planning Committee, the Finance Committee, and the Fundraising Committee.  The Bylaws also provide for two Special Committees: the Strategic Planning Committee and the Nominations Committee.  Additionally, the Society President has established a Scholarship Committee, a Governance Committee, and a new Public Relations/Communications Committee.  If you’re interested in serving on any of these committees, please indicate which committee(s) are of interest to you in the comments block on the form below.

PROGRAMS/PROJECTS: The Society also has opportunities to volunteer/participate in a number of programs/projects we support which include the Maternity Outreach for Veteran Families Program (administered by Veterans First Ltd), the State Veteran Home Outreach Program, the Veterans Treatment Courts, and the Regional Member Outreach Program.  We also provide volunteers for several events throughout the year such as the Carefree Art & Wine Festivals supported by Imprints of Honor (formerly Veterans Heritage Project) and Wreaths Across America.  Other one-time, one-day volunteer opportunities may become available from time to time.

FLAGS FOR FAMILIES HONORS PROGRAM: We are also asking that you provide the Society with your next of kin or survivor contact information in order to assist and facilitate the Society’s presentation of an Arizona Flag and AVHOFS Citation to your family when you pass.  NOTE:  In addition to providing your Next of Kin Contact Information to the Society for our Flags for Families Honors Program, we also encourage you to download, print and file a copy of the document below with your estate documents or provide it to your Next of Kin so that they can contact the Society upon your passing.
