The Arizona Veterans Hall of Fame Society seeks to recognize non-veterans whose extraordinary personal actions contribute significantly to the benefit and well-being of our military, our veterans, and their families. The Copper Sword award is presented at the annual Patriotic Awards Luncheon. Only the nominator will be notified if this nominee is NOT selected.

I certify that the nominee has been established in the State of Arizona for a minimum of two years.

I understand that if selected, a high-resolution color portrait is required within 2 weeks of notification.

I understand the annual deadline for submission of the Copper Sword Award is October 31st.

The nomination includes a document from the Nominator (not to exceed two pages) describing the reason(s) why the nominee is deserving of the Arizona Veterans Hall of Fame Copper Sword Award.

I have included supporting documentation or letters of recommendation.

I understand the total nomination packet will not exceed fifteen pages, including photographs.
